Your Initial Office Visit

Dr. Nigalye will explain your diagnosis and treatment options during your appointment.

A unique treatment plan will be created based on your individual needs and concerns.

A medical clearance may be necessary before treatment, depending on your medical history.

Please assist us by providing the following information before your appointment:

Your surgical referral slip and x-rays, if applicable.

A list of medications you are currently taking.

Your dental benefit plan ID card(s), if applicable, to save time and allow us to submit claims promptly.

To register online, click 'Patient Information' and then 'Patient Registration.' Please fill out the form and click 'Complete and Send' to submit your information to us.

IMPORTANT:

A parent or guardian must accompany patients under 18 years of age to all appointments. A preoperative evaluation and physical examination are mandatory for patients undergoing IV anesthesia for surgery. Please refrain from eating or drinking anything for eight hours before surgery. You will also need a responsible adult to drive you home.


Please inform the office if you have any medical conditions that may be of concern before surgery (e.g., diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, etc.) or if you are currently taking any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.).
